HIV RISK self-ASSESSMENT SURVEY
This survey is designed to assess your potential risk level for HIV and STIs.
There are dozens of factors that affect one’s level of risk and likelihood of HIV and/or STI infection. This survey will help you evaluate your own potential risks; while helping you explore other options and/or ideas surrounding sex.
Project HOPE test counselors are available to assist you with a more personalized risk assessment, to help you plan strategies to reduce your risk, as well as provide HIV/STI/HCV testing. Answer the following seven questions to rate your potential risk for HIV/STI exposure from your own personal sexual history. At the end of the questions, a meter will appear and gauge your potential risk; low to moderate, moderate to moderately high, and high to very high.
